This new form is to be used when a client reports no assets and the new form is to be used effective immediately. Please note: If a client has assets, continue to use Form HCA-160, Verification of Assets, which is currently in each of the toolkits. The Verification of Zero Asset form will be incorporated into the respective toolkits in the future, if a new revision is necessary. Please note: it is not recommended to download forms from the HCA Help Desk and save for future use because changes to forms occur on a regular basis and downloading a form does not allow one to obtain the most up-to-date version from the HCA Help Desk. Not using the most current form could jeopardize an agency's compliance with program requirements.
